Yes, insurance companies deny benefits. If a service isn't covered under the contract, then they have every right to deny paying for it. Even Medicare denies paying for services that aren't covered. What you don't seem to want to see is that they also pay out billions a year for services and medications.

Even services that are covered they refuse or stall paying, requiring patients and the actual treatment providers to jump through hoops for.  

Insurance companies take in much more than they pay out.  When you say, "You can't blame all the problems with health care in the US on the insurance industry", I say of course not.  But even the non-profits make enough money to pay their CEOs over seven figures a year.  

Always remember that insurance companies are in business to make money.  Government administered programs, like Medicare and Social Security, aren't and do a better job for less, with that profit going into patient care.

Hospitals can't refuse emergency treatment. They can, and do, refuse non-emergency treatment to people who either don't have insurance or can't afford to pay in advance for the services needed.
